Each patient receives a tailored treatment plan put together specifically for the individual by our multidisciplinary team.

Assessments

Our referral process starts with a single call to the Clinical Liaison Hotline. Upon receiving your call, your Clinical Liaison will:

• Conduct initial phone screening
• Handle paperwork/documentation copying or faxing
• Make necessary phone calls to family and/or powers of attorney
• Arrange transportation
• Provide post-discharge follow-ups
